I don’t require a doctors referral to assess you however some insurance companies still require it. Please check with your insurance company provider if you are unsure.
Unfortunately, physiotherapy is not covered under OHIP at this clinic.
I do not direct bill insurance, however, I will provide you with an invoice immediately following the session that you can submit to insurance for reimbursement. I accept various forms of payment including cash, e-transfer, Debit, VISA, MC or AMEX.
Robust cleaning protocols are adhered to during and after each session. Masking is optional and at the discretion of the client. Clients are expected to self monitor symptoms and re-schedule appointments pending new or worsening symptoms.
Clients complete an intake form prior to their visit which I review before arrival. The assessment consists of a thorough review of your medical history and current symptoms as well as a discussion about your goals. I then perform a physical assessment and based on findings, discuss techniques and strategies that I may provide in clinic to assist you towards your goals, as well as education and information on what you can do to help yourself!
The frequency of visits is individual and dependent on each client’s needs/goals. You will be frequently reassessed and your plan progressed as able. My goal is to always empower the client and have them be as independent as able in their rehab.
Monday & Wednesday 8:00-5:00
Tuesday & Thursday 9:30-6:00
Friday 8:00-2:00
Note: The clinic is closed between 12:00-1:30 Monday - Thurs.
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a vacancy that could have been filled by another patient. As such, I require 24 hours notice for any cancellations or changes to your appointment, no exceptions. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a 100% cancellation fee to be paid prior to re-booking.
